All audience members must submit self-questionnaires before entering the auditorium
This questionnaire is to comply with the ‘Prevention Guidelines of Infectious Diseases for Theatres’ and is collected from all audiences. The collected personal information is used only for epidemiological tracing in the event of a confirmed patient.

.Questionnaire is available at the theatre and please submit the filled in questionnaire when you enter the auditorium.
. The questionnaire can only be submitted at the theatre on the day of your show observation.
.Admission to the auditorium may be restricted depending on your response on the questionnaire and your body temperature on the day of the performance.

-Do you have fever above 37.5℃ or breathing difficulties?
-Were you advised to self-isolate from the health authority within 15 days as of today, or do you have any family members or housemates who were advised to self-isolate within 15 days as of today?
- Have you traveled to any foreign country within 15 days as of today?
*The questionnaire will be destroyed two weeks after the performance.
